To access the Admission Forms from the State of Utah, please visit their site HERE and click on "Admission Agreement and Health Assessment." (top of the page, right above approved menu)
You may list all of your children on one "admissions" form, we just need separate "child health assessment" forms. Please include your child's IMMUNIZATION record or a signed EXEMPTION form with your registration, Utah State Law Requires this Before a child can be cared for at a licensed facility.
